On-going Project

認知症のリスクをもつ高齢者に対する進展予防を目指した多因子介入によるランダム化比較研究(J-MINT研究)このリンクは別ウィンドウで開きます

高齢者の方に生活習慣病の管理、運動、栄養、認知トレーニングに関する指導を受けていただき生活習慣を改善することで、認知機能低下を抑制できるかどうかを明らかにすることが主な目的です。国内の5施設で研究を行い、全体で約500名の方に参加いただいています。

軽度認知障害の人における進行予防と精神心理的支援のための手引き作成と介入研究(厚生労働省科学研究)

軽度認知障害(MCI)は、認知症の前段階と考えられ、この段階から認知症への進行を予防するために生活習慣(運動や食事)を見直すことが大切です。しかし、MCIの段階の人への支援は十分に行われているとは言い難いのが現状です。私たちは、MCIに関するわかりやすい『手引き(厚生労働省科学研究版)』を作成し、MCIの段階から支援をするためのプログラムの開発が必要と考えています。この研究では、高齢者の方に生活習慣病の管理、運動、栄養、認知トレーニングに関する手引きを用いたプログラムを受けていただき、より適切な生活習慣が身につくか(行動変容と呼びます)を明らかにすることが主な目的です。

地域在住高齢者を対象とした多因子介入による認知症予防プログラムの実現可能性の検討

認知症のリスクを持つ高齢者を対象とした、認知症予防のための多因子介入プログラム(J-MINT研究)を地域社会で実装するために、1)地域の文脈へ適応(Adaptation)された多因子介入プログラムが適切に運用できるかの実施可能性を確認するとともに、2)自治体が主体となり地域で実施するための実装戦略の開発を行います。令和5年度は、大府市との共同研究で2つの地域(東山地区と吉田地区)で行われます。

その他

国立長寿医療研究センター内(コホート研究、バイオバンク事業、臨床データベースの構築等)、国立高度専門医療研究センター内、大学や企業との共同研究が多数進行中です。

 軽度認知障害(MCI)は、認知症の前段階と考えられ、この段階から認知症への進行を予防するために生活習慣(運動や食事)を見直すことが大切です。しかし、MCIの段階の人への支援は十分に行われているとは言い難いのが現状です。私たちは、MCIに関するわかりやすい手引き『あたまとからだを元気にするMCIハンドブック(厚生労働省科学研究版)』その補足資料として『生活ノート』を作成しました。

J-MINT研究

認知症のリスクをもつ高齢者に対する進展予防を目指した多因子介入によるランダム化比較研究(J-MINT研究)

J-MINT研究とは?

 高齢化とともに認知症の人の数も増加しています。認知症は要介護状態になる原因の第一位となっており、認知症を予防するための適切な対策を立てることが重要な課題となっています。認知症の予防については、糖尿病、高血圧、肥満などが認知症の危険因子であることが明らかとなるとともに、フィンランドで行われたFINGER研究によって、運動や栄養改善に加えて、認知トレーニングを行うといった多因子介入により、認知機能低下のスピードが緩やかになることが示されました。

 我が国でも、多因子介入の認知機能低下に対する予防効果を検証するために、我が国独自の取り組みを加えた介入研究であるJ-MINT研究が、2019年、国立長寿医療研究センターを中心に開始されました。J-MINT研究には、同年代の方と比較して少し認知機能が低下している高齢者531名が研究に参加し、多因子介入プログラム (生活習慣病の管理、運動、栄養、認知トレーニング) を受けるグループと受けないグループにランダムに分かれて、18ヵ月間の認知機能の変化を比較しました。

 J-MINT研究は、COVID-19の感染拡大の影響を受け、一時は介入プログラムの提供を中断せざるを得ない状況となりました。しかし、オンラインで運動教室を開催するなど対策を行いながら、2022年12月に全対象者の介入プログラムの提供と評価を終えることができました。

 全体の解析では、18ヵ月間の認知機能の変化に差はありませんでしたが、運動教室に70%以上参加していたグループでは、70%未満のグループ、多因子介入プログラムを受けないグループと比較して、認知機能が改善していたことが示されました。

 J-MINT研究は、現在も1年毎に追跡調査を行っており、多因子介入プログラムの長期的な効果を検証しています。また、地域の皆様と一緒に認知症予防のための多因子介入プログラムを社会に広める活動にも取り組んでいます。

NJ-FINGER研究とは?

 Nordic-Japan Multidomain Intervention for Healthy Aging and Prevention of Dementia and Disability (NJ-FINGER)は、認知症および認知機能障害の予防に関する、北欧3カ国(フィンランド、ノルウェー、スウェーデン)と日本の研究者によって組織される国際プロジェクトです。認知症予防を目指した多因子介入の先駆けとなったFINGER研究と、日本で行われたJ-MINT研究の知見を統合し、健康長寿社会の実現に役立つ新しい科学的知見を創出することを目的としています。

2022年7月にオランダ・アムステルダムで開催された世界認知症学会(The Alzheimer's Association International Conference)に合わせてNJ-FINGER研究の国際会議を行いました。荒井理事長、櫻井所長、杉本研究員、藤田研究員はMiia Kivipelto教授らと、認知症データベースの構築と認知症リスクのNordic-Japan比較について議論し、統合データベースの作成を進めることとしました。

2023年12月にカロリンスカ研究所のMiia Kivipelto教授とFinnish Institute for Health and WelfareのTiia Ngandu准教授らのチームがNCGGを訪問しました。NJ-FINGER研究で取り組んでいる統合データベース整備の進捗状況の確認と日本と北欧での認知症リスクの比較についてディスカッションを行うとともに、Miia Kivipelto教授が日本の研究者に向けて、Advancing precision prevention of dementia through global collaboration: World-Wide FINGERSと題した基調講演を行いました。
